Application for Contracting (Groups of 3 or more)
This option is for contracting with THREE or MORE insurance payers under a company group policy. If you are only applying to one or two networks, please use one of  the standard credentialing option instead. The Credentialing Bundle includes contracting for multiple insurance payers at a discounted rate and the individual applications as a courtesy.

Select each payer you need from the dropdown menu and add it to your cart. Repeat this process for each additional payer you're applying to.
