Application for Contracting (Group Organizations)

This option is for contracting with ONE or TWO insurance payers under a company group policy. If you are applying to three or more networks, please use the Group Contracting Bundle option instead, which offers greater savings. This option includes group contracting for up to two insurance payers.

The individual application fee is added at a discounted rate of $75 per provider (regularly $165). Select each payer you need from the dropdown menu and add it to your cart. Repeat this process for the second payer if applicable (up to 2 payers total).
